need to complete and return this medical
form by Jan 28, 2010. This form was emailed with
a notice of your registration confirmation.
Scholarship Application
Form (pdf) - If you need
financial assistance to be able to attend
Confirmation Camp, please complete and return
this form to the camp as soon as possible.
Please note that it will need to be signed by
your pastor. Our normal practice is to award
scholarships equal to 1/3 of the earlybird fee.
