The following forms are available for the Confirmation Camp

Registration Form (pdf)for those who do not want to use the online registration process. This form can be filled out on your computer and then printed and signed. (Note:You cannot save the completed form to your computer or email it.)

Camper Health History Form (pdf) - Each confirmand will need to complete and return this medical form prior to the Confirmation Camp or bring it with you to check-in. This form was emailed with a notice of your registration confirmation. If you cannot locate it, you can download it here.

Scholarship Application Form (pdf) - If you need financial assistance to be able to attend Confirmation Camp, please complete and return this form to the camp as soon as possible. Please note that it will need to be signed by your pastor. Our normal practice is to award scholarships equal to 1/3 of the earlybird fee. If you have questions, contact the office at 830-625-2212.
